E-SIGN Disclosure and Consent

Consent to Electronic Records

ShapeCard is an electronic service. To apply for a ShapeCard card and to keep an account, you must agree to receive records from us electronically instead of on paper. This disclosure explains what that means and your rights. Please read it, and save or print a copy for your records.

What This Consent Covers

Your consent applies to your application and to every ShapeCard account and card it opens, whether a debit card or a credit card, for as long as you have one. It covers all records we or our bank partner are required to give you in writing, including:

• Your application, the disclosures shown with it, and any notice about its result
• Your cardholder agreement, fee and rate disclosures, and any change to their terms
• Account statements, transaction receipts and error resolution notices
• Privacy notices and other notices required by law
• Records about a card you apply for or manage for someone else

A supervised adult who holds their own account gives their own consent. Your consent covers the records sent to you as their approver.

How We Deliver Records

We deliver records in one or more of these ways:

• By email to the address on your application or account
• By posting them in your ShapeCard portal or app and telling you they are there
• By showing them on screen during the application, where you can save or print them

Text messages may tell you a record is ready, but a text message is never the only way we deliver a record.

Your Right to Paper Copies

You may ask for a paper copy of any record we provide electronically, at no charge, by emailing support@shapecard.ai or by writing to ShapeCard LLC, 18 Timberline Rd, Norwalk, CT 06854. Tell us which record you want. Asking for a paper copy does not withdraw your consent.

Withdrawing Your Consent

You may withdraw your consent at any time, at no charge, by emailing support@shapecard.ai. Withdrawal takes effect after we have had a reasonable time to process it, and it does not change the legal effect of records we delivered before then.

Because ShapeCard is offered only electronically, if you withdraw before your account opens we will not be able to continue your application, and if you withdraw after it opens we may close your account and cards. We will tell you before we do.

Keeping Your Contact Information Current

You must keep your email address and phone number current so records reach you. You can update them in your ShapeCard account settings or by emailing support@shapecard.ai.

Hardware and Software You Need

To open and keep electronic records you need:

• A computer, phone or tablet with internet access
• A current version of Chrome, Safari, Firefox or Edge, with cookies and JavaScript turned on
• Software that opens PDF files
• An email account that can receive messages from shapecard.ai
• Storage space to save records, or a printer to print them

If these requirements change in a way that could stop you from opening or keeping a record, we will tell you first, and you may withdraw your consent without any fee.

Your Consent

You give this consent by checking a box when you apply. Checking it is your electronic signature, and it shows that you can open and read records in the form we will use, because you are reading this one now. The card application shows its own box for this consent, with the statement below.

"I have read the E-SIGN Disclosure and Consent. I agree to receive this application and all records, disclosures and notices about my ShapeCard account electronically. I understand I can ask for a free paper copy and can withdraw this consent at any time. I confirm I was able to open and read this disclosure on this device, and that I can save or print it."
